Welcome to the Lebanon County LINK to Aging and Disability Resources
The Lebanon County Link to Aging and Disability Resources is your one-stop, “no-wrong door” resource for persons over age 60 and persons with disabilities – consumers, family members and others seeking information about local resources.
Call or contact one or more of these core (below) or collaborative partner organizations on this Website.
The Lebanon County LINK to Aging and Disability Resources is your seamless network of Core and Collaborative Partner agencies which allows you to initiate contact with any Link Network partner and, generally speaking, experience the same intake to access information, assistance, and resources.
All consumers and citizens seeking information about services must be, directly or seamlessly, connected to the information and services needed, no matter the initial point of entry into the system.
The “no wrong door” approach attempts to minimize duplication of efforts by consumer and providers, minimizes potential confusion and steps a consumer needs to complete to receive services, enhance individual choice, support informed decision-making, and increase the cost effectiveness of long term support systems.
If you are a person over age 60 or a person with a disability between the ages of 18 to 59, the Lebanon County LINK to Aging and Disability Resources’ core and collaborative partnership can help you:
- Evaluate services necessary to remain independent
- Make connections with organizations that provide needed services
- Assist with applications to determine funding eligibility
